Temporal Lobe Resection Glossary of Terms

The following are health and medical definitions of terms that appear in the Temporal Lobe Resection article.

Allergic reaction: The hypersensitive response of the immune system of an allergic individual to a substance.

Anesthesia: Loss of feeling or awareness. A general anesthetic puts the person to sleep. A local anesthetic causes loss of feeling in a part of the body such as a tooth or an area of skin without affecting consciousness. Regional anesthesia numbs a larger part of the body such as a leg or arm, also without affecting consciousness. The term "conduction anesthesia" encompasses both local and regional anesthetic techniques. Many surgical procedures can be done with conduction anesthesia without significant pain. In many situations, such as a C-section, conduction anesthesia is safer and therefore preferable to general anesthesia. However, there are also many types of surgery in which general anesthesia is clearly appropriate.

Anterior: The front, as opposed to the posterior. The anterior surface of the heart is toward the breast bone (the sternum).

Brain: That part of the central nervous system that is located within the cranium (skull). The brain functions as the primary receiver, organizer and distributor of information for the body. It has two (right and left) halves called "hemispheres."

Cerebrum: The largest part of the brain. It is divided into two hemispheres, or halves.

Complication: In medicine, an additional problem that arises following a procedure, treatment or illness and is secondary to it. A complication complicates the situation.

Craniotomy: A surgical operation in which an opening is made in the skull.

Dura: The outermost, toughest, and most fibrous of the three membranes (meninges) covering the brain and the spinal cord. Dura is short for dura mater (from the Latin for hard mother). Also called the pachymeninx (singular) or pachymeniges (plural).

Ear: The hearing organ. There are three sections of the ear, according to the anatomy textbooks. They are the outer ear (the part we see along the sides of our head behind the temples), the middle ear, and the inner ear. But in terms of function, the ear has four parts: those three and the brain. Hearing thus involves all parts of the ear as well as the auditory cortex of the brain. The external ear helps concentrate the vibrations of air on the ear drum and make it vibrate. These vibrations are transmitted by a chain of little bones in the middle ear to the inner ear. There they stimulate the fibers of the auditory nerve to transmit impulses to the brain.

EEG: Electroencephalogram, e technique for studying the electrical current within the brain. Electrodes are attached to the scalp. Wires attach these electrodes to a machine which records the electrical impulses. The results are either printed out or displayed on a computer screen. Electroencephalogram is abbreviated EEG.

Epilepsy (seizure disorder): When nerve cells in the brain fire electrical impulses at a rate of up to four times higher than normal, this causes a sort of electrical storm in the brain, known as a seizure. A pattern of repeated seizures is referred to as epilepsy. Known causes include head injuries, brain tumors, lead poisoning, maldevelopment of the brain, genetic and infectious illnesses. But in fully half of cases, no cause can be found. Medication controls seizures for the majority of patients.

Frontal: In anatomy, pertaining to the forehead. As, for example, the frontal bone, frontal sinus, and frontal nerve. From the Latin "frons" meaning the forehead or brow.

Incision: A cut. When making an incision, a surgeon is making a cut.

Infection: The growth of a parasitic organism within the body. (A parasitic organism is one that lives on or in another organism and draws its nourishment therefrom.) A person with an infection has another organism (a "germ") growing within him, drawing its nourishment from the person.

Lobe: Part of an organ that appears to be separate in some way from the rest. A lobe may be demarcated from the rest of the organ by a fissure (crack), sulcus (groove), connective tissue or simply by its shape. For example, there are the frontal, parietal, temporal, and occipital lobes of the brain.

Magnetic resonance imaging: A special radiology technique designed to image internal structures of the body using magnetism, radio waves, and a computer to produce the images of body structures. In magnetic resonance imaging (MRI), the scanner is a tube surrounded by a giant circular magnet. The patient is placed on a moveable bed that is inserted into the magnet. The magnet creates a strong magnetic field that aligns the protons of hydrogen atoms, which are then exposed to a beam of radio waves. This spins the various protons of the body, and they produce a faint signal that is detected by the receiver portion of the MRI scanner. A computer processes the receiver information, and an image is produced. The image and resolution is quite detailed and can detect tiny changes of structures within the body, particularly in the soft tissue, brain and spinal cord, abdomen and joints.

Membrane: A very thin layer of tissue that covers a surface.

Memory: 1. The ability to recover information about past events or knowledge. 2. The process of recovering information about past events or knowledge. 3. Cognitive reconstruction. The brain engages in a remarkable reshuffling process in an attempt to extract what is general and what is particular about each passing moment.

Mesial: 1. Toward the middle. As, for example, a mesial temporal lobe structure.
2. In dentistry, toward the middle of the front of the jaw.

MRI: Abbreviation and nickname for magnetic resonance imaging. For more information, see: Magnetic Resonance Imaging; Paul C. Lauterbur; Peter Mansfield.

Nausea: Nausea, is the urge to vomit. It can be brought by many causes including, systemic illnesses, such as influenza, medications, pain, and inner ear disease. When nausea and/or vomiting are persistent, or when they are accompanied by other severe symptoms such as abdominal pain, jaundice, fever, or bleeding, a physician should be consulted.

Occipital: 1. Pertaining to the occiput, the back of head.
2. Located near the occipital bone as, for example, the occipital lobe of the brain.

Pain: An unpleasant sensation that can range from mild, localized discomfort to agony. Pain has both physical and emotional components. The physical part of pain results from nerve stimulation. Pain may be contained to a discrete area, as in an injury, or it can be more diffuse, as in disorders like fibromyalgia. Pain is mediated by specific nerve fibers that carry the pain impulses to the brain where their conscious appreciation may be modified by many factors.

Parietal: Adjective from the Latin "parietalis" meaning "belonging to the wall" that the ancient anatomists used to designate the wall, as of a body cavity.

Positron emission tomography: PET. A highly specialized imaging technique that uses short-lived radioactive substances to produce three-dimensional colored images of those substances functioning within the body. These images are called PET scans and the technique is termed PET scanning.

Quality of life: An important consideration in medical care, quality of life refers to the patient's ability to enjoy normal life activities. Some medical treatments can seriously impair quality of life without providing appreciable benefit, while others greatly enhance quality of life.

Resection: Surgical removal of part of an organ.

Seizure: Uncontrolled electrical activity in the brain, which may produce a physical convulsion, minor physical signs, thought disturbances, or a combination of symptoms.

Sensation: In medicine and physiology, sensation refers to the registration of an incoming (afferent) nerve impulse in that part of the brain called the sensorium, which is capable of such perception. Therefore, the awareness of a stimulus as a result of its perception by sensory receptors. (Sensory is here synonymous with sensation.)

Skull: The skull is a collection of bones which encase the brain and give form to the head and face. The bones of the skull include the following: the frontal, parietal, occipital, temporal, sphenoid, ethmoid, zygomatic, maxilla, nasal, vomer, palatine, inferior concha, and mandible.

Surgeon: A physician who treats disease, injury, or deformity by operative or manual methods. A medical doctor specialized in the removal of organs, masses and tumors and in doing other procedures using a knife (scalpel). The definition of a "surgeon" has begun to blur in recent years as surgeons have begun to minimize the cutting, employ new technologies that are "minimally invasive," use scopes, etc.

Surgery: The word "surgery" has multiple meanings. It is the branch of medicine concerned with diseases and conditions which require or are amenable to operative procedures. Surgery is the work done by a surgeon. By analogy, the work of an editor wielding his pen as a scalpel is s form of surgery. A surgery in England (and some other countries) is a physician's or dentist's office.

Temporal: 1. Pertaining to time, limited in time, temporary, or transient.
2. Pertaining to the temple region of the head. The temporal lobe of the brain is located beneath the temple.
From the Latin tempus which means both time and the temple of the head.

Temporal lobe: The lobe of the cerebral hemisphere located down on the side just forward of the occipital lobe. The temporal lobe contains the auditory cortex which is responsible for hearing. It is also the site of the seizure activity characteristic of temporal-lobe epilepsy.

Tired: A feeling of a lessened capacity for work and reduced efficiency of accomplishment, usually accompanied by a sense of weariness and fatigue.

Tomography: The process for generating a tomogram, a two-dimensional image of a slice or section through a three-dimensional object. Tomography achieves this remarkable result by simply moving an x-ray source in one direction as the x-ray film is moved in the opposite direction during the exposure to sharpen structures in the focal plane, while structures in other planes appear blurred. The tomogram is the picture; the tomograph is the apparatus; and tomography is the process.

What Is Multiple Subpial Transection?

Sometimes brain seizures begin in a vital area of the brain -- for example, in areas that control movement, feeling, language, or memory. When this is the case, a relatively new epilepsy treatment called multiple subpial transection (MST) may be an option. MST stops the seizure impulses by cutting nerve fibers in the outer layers of the brain (gray matter), sparing the vital functions concentrated in the deeper layers of brain tissue (white matter).

Who Is a Candidate for Multiple Subpial Transection?

Most people with epilepsy can control their seizures with medication. However, about 20% of people with epilepsy do not improve with drugs. In some cases, surgery to remove the part of the brain causing the seizures may be recommended.
