Epidural Steroid Injection Glossary of Terms

The following are health and medical definitions of terms that appear in the Epidural Steroid Injection article.

Anatomy: The study of form. Gross anatomy involves structures that can be seen with the naked eye. It is as opposed to microscopic anatomy (or histology) which involves structures seen under the microscope. Traditionally, both gross and microscopic anatomy have been studied in the first year of medical school in the U.S. The most celebrated textbook of anatomy in the English-speaking world is Gray's Anatomy, still a useful reference book. The word "anatomy" comes from the Greek ana- meaning up or through + tome meaning a cutting. Anatomy was once a "cutting up" because the structure of the body was originally learned through dissecting it, cutting it up. The abbreviation for anatomy is anat.

Cerebrospinal fluid: CSF. A watery fluid, continuously produced and absorbed, which flows in the ventricles (cavities) within the brain and around the surface of the brain and spinal cord.

Cervical: Having to do with any kind of neck including the neck on which the head is perched and the neck of the uterus. The word "cervix" in Latin means "neck". That is why cervical vertebrae and cervical cancer involve quite disparate parts of the anatomy joined only by the meaning of the word "cervix".

Compression: 1. The act of pressing together. As in a compression fracture, nerve compression, or spinal cord compression.
2. To shorten in time. In embryology, there may be compression of development with some stages even omitted.

Disc: Shortened terminology for an intervertebral disc, a disk-shaped piece of specialized tissue that separates the bones of the spinal column.

Dura: The outermost, toughest, and most fibrous of the three membranes (meninges) covering the brain and the spinal cord. Dura is short for dura mater (from the Latin for hard mother). Also called the pachymeninx (singular) or pachymeniges (plural).

Epidural: Outside the dura, the outermost, toughest, and most fibrous of the three membranes (meninges) covering the brain and the spinal cord. An epidural hematoma is a collection of blood beneath the skull but outside the dura.

Herniation: Abnormal protrusion of tissue through an opening. For example, a intervertebral disk (one situated between the vertebral bodies) can protrude and impinge on a nerve root.

Leg: In popular usage, the leg extends from the top of the thigh down to the foot. However, in medical terminology, the leg refers to the portion of the lower extremity from the knee to the ankle.

Lumbar: Referring to the 5 lumbar vertebrae which are situated below the thoracic vertebrae and above the sacral vertebrae in the spinal column. The 5 lumbar vertebrae are represented by the symbols L1 through L5. There are correspondingly 5 lumbar nerves.

Lumbar radiculopathy: Nerve irritation caused by damage to the discs between the vertebrae. Damage to the disc occurs because of degeneration ("wear and tear") of the outer ring of the disc, traumatic injury, or both. As a result, the central softer portion of the disc can rupture (herniate) through the outer ring of the disc and abut the spinal cord or its nerves as they exit the bony spinal column. This rupture is what causes the commonly recognized pain of "sciatica" that shoots down the leg.

Membrane: A very thin layer of tissue that covers a surface.

Neck: The part of the body joining the head to the shoulders. Also, any narrow or constricted part of a bone or organ that joins its parts as, for example, the neck of the femur bone.

Nerve: A bundle of fibers that uses chemical and electrical signals to transmit sensory and motor information from one body part to another. See: Nervous system.

Nerve compression: "Pinching" a nerve by putting too much pressure on it. For example, the sciatic nerve may be painfully compressed by a ruptured disc in the lower spine causing sciatica.

Pain: An unpleasant sensation that can range from mild, localized discomfort to agony. Pain has both physical and emotional components. The physical part of pain results from nerve stimulation. Pain may be contained to a discrete area, as in an injury, or it can be more diffuse, as in disorders like fibromyalgia. Pain is mediated by specific nerve fibers that carry the pain impulses to the brain where their conscious appreciation may be modified by many factors.

Radiculopathy: Any disease of the spinal nerve roots and spinal nerves. It is synonymous with radiculitis.

Shock: In medicine, shock is a critical condition brought on by a sudden drop in blood flow through the body. There is failure of the circulatory system to maintain adequate blood flow. This sharply curtails the delivery of oxygen and nutrients to vital organs. It also compromises the kidney and so curtails the removal of wastes from the body. Shock can be due to a number of different mechanisms including not enough blood volume (hypovolemic shock) and not enough output of blood by the heart (cardiogenic shock). The signs and symptoms of shock include low blood pressure (hypotension), overbreathing (hyperventilation), a weak rapid pulse, cold clammy grayish-bluish (cyanotic) skin, decreased urine flow (oliguria), and mental changes (a sense of great anxiety and foreboding, confusion and, sometimes, combativeness).

Spinal cord: The major column of nerve tissue that is connected to the brain and lies within the vertebral canal and from which the spinal nerves emerge. Thirty-one pairs of spinal nerves originate in the spinal cord: 8 cervical, 12 thoracic, 5 lumbar, 5 sacral, and 1 coccygeal. The spinal cord and the brain constitute the central nervous system (CNS). The spinal cord consists of nerve fibers that transmit impulses to and from the brain. Like the brain, the spinal cord is covered by three connective-tissue envelopes called the meninges. The space between the outer and middle envelopes is filled with cerebrospinal fluid (CSF), a clear colorless fluid that cushions the spinal cord against jarring shock. Also known simply as the cord.

Spinal nerve: One of the nerves that originates in the spinal cord.

Spine: 1) The column of bone known as the vertebral column, which surrounds and protects the spinal cord. The spine can be categorized according to level of the body: i.e., cervical spine (neck), thoracic spine (upper and middle back), and lumbar spine (lower back). See also vertebral column. 2) Any short prominence of bone. The spines of the vertebrae protrude at the base of the back of the neck and in the middle of the back. These spines protect the spinal cord from injury from behind.

Steroid: A general class of chemical substances that are structurally related to one another and share the same chemical skeleton (a tetracyclic cyclopenta[a]phenanthrene skeleton).

Syringe: A device used in medicine to inject fluid into or withdraw fluid from the body. Medical syringes consist of a needle attached to a hollow cylinder that is fitted with a sliding plunger. The downward movement of the plunger injects fluid; upward movement withdraws fluid.

An MRI or CT scan is performed to diagnose a herniated disc. Treatment may involve physical therapy, cortisone injection, pain medications, antiinflammatory medications, muscle relaxants, and surgery.
    • Lumbar Spinal Stenosis
      • Lumbar stenosis can be caused by degenerative arthritis (the most common cause), tumor, infection, or metabolic disorders (Paget's disease of the bone). Symptoms include low back pain, weakness, pain, numbness, and loss of sensation in the legs. Other conditions may cause similar symptoms of lumbar stenosis, including diabetic neuropathy, claudication, and peripheral vascular disease. Diagnosis, is a medical history and imaging studies. Lumbar stenosis may be treated with medication or surgery.
    • Radiculopathy
      • Radiculopathy, a condition in which a nerve or nerves along the spine are compressed causing pain, numbness, weakenss, and tingling along the nerve(s). Some causes of radiculopathy include bone spurs, disc hernation, osteoarthritis, tumors, infection, and neuropathy. Treatment depends on the are of nerve compression. Surgery is generally not required.
    • Neuropathic Pain
      • Neuropathic pain is chronic pain resulting from injury to the nervous system. The injury can be to the central nervous system (brain and spinal cord) or the peripheral nervous system (nerves outside the brain and spinal cord).
    • Spondylolisthesis
      • The five types of spondylolisthesis include 1) dysplastic, 2) isthmic, 3) degenerative, 4) traumatic, and 5) pathologic. The most common symptom of spondylolisthesis is lower back pain. Treatment depends on the type and severity of spondylolisthesis. Surgery is required in some cases of spondylolisthesis.

      • Pain management and treatment can be simple or complex, according to its cause. There are two basic types of pain, nociceptive pain and neuropathic pain.     • Spinal Cord Injury: Treatments and Rehabilitation
      • When vertebrae are broken or dislocated, the result can cause traumatic injury to the spinal cord. A spinal cord injury can have significant physiological consequences. One indication of the severity of a spinal cord injury are respiratory complications. Spinal cord injuries are classified as either complete or incomplete. A spinal cord injury can affect breathing, lead to pneumonia, low blood pressure, irregular heart beat, blood clots, spasms, autonomic dysreflexia, bed sores (pressure sores), chronic pain, bladder and bowel problems, and reproductive and sexual function issues. Rehabilitation and recovery of a spinal cord injury is dependant upon the type of injury.

What is radiculopathy?

Radiculopathy is a condition due to a compressed nerve in the spine that can cause pain, numbness, tingling, or weakness along the course of the nerve. Radiculopathy can occur in any part of the spine, but it is most common in the lower back (lumbar radiculopathy) and in the neck (cervical radiculopathy). It is less commonly found in the middle portion of the spine (thoracic radiculopathy).

What are the risk factors for radiculopathy?

Risk factors for radiculopathy are activities that place an excessive or repetitive load on the spine. Patients involved in heavy labor or contact sports are more prone to develop radiculopathy than those with a more sedentary lifestyle. A family history of radiculopathy or other spine disorders also increase the risk of developing radiculopathy.
