Endotracheal Intubation Glossary of Terms

The following are health and medical definitions of terms that appear in the Endotracheal Intubation article.

Adam's apple: A familiar anatomic feature in the front of the neck that is due to the forward protrusion of the thyroid cartilage, the largest and most prominent cartilage of the larynx.

Airway: The path air follows to get into and out of the lungs. The mouth and nose are the normal entry and exit ports. Entering air then passes through the back of the throat (pharynx), continues through the voice box (larynx), down the trachea, and finally out the branching tubes known as bronchi.

ARDS: Acute Respiratory Distress Syndrome. A fulminant lung condition in which trauma to the lungs leads to inflammation of the lungs, accumulation of fluid in the alveolar air sacs, low blood oxygen, and respiratory distress.

Aspiration: Removal of a sample of fluid and cells through a needle. Aspiration also refers to the accidental sucking in of food particles or fluids into the lungs.

Brain: That part of the central nervous system that is located within the cranium (skull). The brain functions as the primary receiver, organizer and distributor of information for the body. It has two (right and left) halves called "hemispheres."

Cardiac: Having to do with the heart.

Cartilage: Firm, rubbery tissue that cushions bones at joints. A more flexible kind of cartilage connects muscles with bones and makes up other parts of the body, such as the larynx and the outside parts of the ears.

Endotracheal tube: A flexible plastic tube that is put in the mouth and then down into the trachea (the airway). The doctor inserts the tube under direct vision with the help of a laryngoscope. The procedure is called endotracheal intubation. The purpose is to ventilate the lungs.

Esophagus: The tube that connects the pharynx (throat) with the stomach. The esophagus lies between the trachea (windpipe) and the spine. It passes down the neck, pierces the diaphragm just to the left of the midline, and joins the cardiac (upper) end of the stomach. In an adult, the esophagus is about 25 centimeters (10 inches) long. When a person swallows, the muscular walls of the esophagus contract to push food down into the stomach. Glands in the lining of the esophagus produce mucus, which keeps the passageway moist and facilitates swallowing. Also known as the gullet or swallowing tube. From the Greek oisophagos, from oisein meaning to bear or carry + phagein, to eat.

Intubation: The process of putting a tube into a hollow organ or passageway, often into the airway. The opposite of intubation is extubation.

Laryngoscope: A flexible, lighted tube used to look at the inside of the larynx (the voice box). The laryngoscope is inserted through the mouth into the upper airway.

Lungs: The lungs are a pair of breathing organs located with the chest which remove carbon dioxide from and bring oxygen to the blood. There is a right and left lung.

Mouth: 1. The upper opening of the digestive tract, beginning with the lips and containing the teeth, gums, and tongue. Foodstuffs are broken down mechanically in the mouth by chewing and saliva is added as a lubricant. Saliva contains amylase, an enzyme that digests starch. 2. Any opening or aperture in the body. The mouth in both senses of the word is also called the os, the Latin word for an opening, or mouth. The o in os is pronounced as in hope. The genitive form of os is oris from which comes the word oral.

Pneumonia: Inflammation of one or both lungs with consolidation. Pneumonia is frequently but not always due to infection. The infection may be bacterial, viral, fungal or parasitic. Symptoms may include fever, chills, cough with sputum production, chest pain, and shortness of breath.

Pneumothorax: Free air in the chest outside the lung.

Respiration: 1. The act of inhaling and exhaling air in order to exchange oxygen for carbon dioxide. Synonymous with breathing and ventilation. 2. The cellular metabolic process by which oxygen is taken in, substances are oxidized, energy is released, and carbon dioxide and oxidized products are given off.

Respiratory: Having to do with respiration, the exchange of oxygen and carbon dioxide. From the Latin re- (again) + spirare (to breathe) = to breathe again.

Stomach: 1. The sac-shaped digestive organ that is located in the upper abdomen, under the ribs. The upper part of the stomach connects to the esophagus, and the lower part leads into the small intestine.

Surgery: The word "surgery" has multiple meanings. It is the branch of medicine concerned with diseases and conditions which require or are amenable to operative procedures. Surgery is the work done by a surgeon. By analogy, the work of an editor wielding his pen as a scalpel is s form of surgery. A surgery in England (and some other countries) is a physician's or dentist's office.

Throat: The throat is the anterior (front) portion of the neck beginning at the back of the mouth, consisting anatomically of the pharynx and larynx. The throat contains the trachea and a portion of the esophagus.

Thyroid: 1. The thyroid gland. Also, pertaining to the thyroid gland. 2. A preparation of the thyroid gland used to treat hypothyroidism. 3. Shaped like a shield. (The thyroid gland was so-named by Thomas Wharton in 1656 because it was shaped like an ancient Greek shield.)

Thyroid cartilage: The largest of the cartilages of the larynx (the voice box).

Tongue: The tongue is a strong muscle anchored to the floor of the mouth. It is covered by the lingual membrane which has special areas to detect tastes.

Trachea: A tube-like portion of the breathing or "respiratory" tract that connects the "voice box" (larynx) with the bronchial parts of the lungs.

Tubes: The "tubes" are medically known as the Fallopian tubes. There are two Fallopian tubes, one on each side, which transport the egg from the ovary to the uterus (the womb). The Fallopian tubes have small hair-like projections called cilia on the cells of the lining.

Unconscious: 1. Interruption of awareness of oneself and one's surroundings, lack of the ability to notice or respond to stimuli in the environment. A person may become unconscious due to oxygen deprivation, shock, central nervous system depressants such as alcohol and drugs, or injury.
2. In psychology, that part of thought and emotion that happens outside everyday awareness.

Ventilation: The exchange of air between the lungs and the atmosphere so that oxygen can be exchanged for carbon dioxide in the alveoli (the tiny air sacs in the lungs).

Ventilator: A ventilator is a machine which mechanically assists patients in the exchange of oxygen and carbon dioxide (sometimes referred to as artificial respiration).

What is a tracheostomy?

A tracheostomy is a surgically created opening in the neck leading directly to the trachea (the breathing tube). It is maintained open with a hollow tube called a tracheostomy tube.

Why is a tracheostomy performed?

A tracheostomy is usually done for one of three reasons: (1) to bypass an obstructed upper airway (an object obstructing the upper airway will prevent oxygen from the mouth to reach the lungs); (2) to clean and remove secretions from the airway; and (3) to more easily, and usually more safely, deliver oxygen to the lungs.
