NOTES: Laboratory and/or medical tests (including heart function tests) may be performed to monitor for side effects and response to treatment.

MISSED DOSE: If you miss a dose or if the dose is interrupted, contact your doctor immediately to establish a new dosing schedule.

STORAGE: Store vials in a refrigerator between 36 and 46 degrees F (2 to 8 degrees C) away from light and moisture. After mixing and dilution in correct IV fluids (consult your pharmacist for details), the drug is stable at refrigerator temperature for six hours. Discard any unused liquid.
