Coma Glossary of Terms

The following are health and medical definitions of terms that appear in the Coma article.

Abdomen: The belly , that part of the body that contains all of the structures between the chest and the pelvis . The abdomen is separated anatomically from the chest by the diaphragm , the powerful muscle spanning the body cavity below the lungs .

Abnormal: Not normal. Deviating from the usual structure, position, condition, or behavior. In referring to a growth, abnormal may mean that it is cancerous or premalignant (likely to become cancer ).

Acetaminophen: A pain reliever and fever reducer. Brand name: Tylenol. The exact mechanism of action of acetaminophen is not known. Acetaminophen relieves pain by elevating the pain threshold (that is, by requiring a greater amount of pain to develop before it is felt by a person). Acetaminophen reduces fever through its action on the heat-regulating center (the "thermostat") of the brain. Generic is available.

Acute: Of abrupt onset, in reference to a disease. Acute often also connotes an illness that is of short duration, rapidly progressive, and in need of urgent care.

Aerobic: Oxygen-requiring. Aerobic bacteria need oxygen to grow. Aerobic exercise requires the heart and lungs to work harder to meet the body's increased oxygen demand.

Airway: The path air follows to get into and out of the lungs. The mouth and nose are the normal entry and exit ports. Entering air then passes through the back of the throat (pharynx), continues through the voice box (larynx), down the trachea, and finally out the branching tubes known as bronchi.

Alcohol: An organic chemical in which one or more hydroxyl (OH) groups are attached to carbon (C) atoms in place of hydrogen (H) atoms. Common alcohols include ethyl alcohol or ethanol (found in alcoholic beverages), methyl alcohol or methanol (can cause blindness) and propyl alcohol or propanol (used as a solvent and antiseptic ). Rubbing alcohol is a mixture of acetone , methyl isobutyl ketone, and ethyl alcohol. In everyday talk, alcohol usually refers to ethanol as, for example, in wine, beer, and liquor. It can cause changes in behavior and be addictive.

Alcoholism: Physical dependence on alcohol to the extent that stopping alcohol use will bring on withdrawal symptoms. In popular and therapeutic parlance, the term may also be used to refer to ingrained drinking habits that cause health or social problems. Treatment requires first ending the physical dependence, then making lifestyle changes that help the individual avoid relapse . In some cases, medication or hospitalization are needed. Alcohol dependence can have many serious effects on the brain , liver , and other organs of the body.

Ammonia: A colorless gas with a very sharp odor. Made both by humans and by nature, ammonia dissolves easily in water and evaporates quickly. Liquid ammonia is found in many household cleaners. Ammonia is irritating to the skin, eyes, nose, throat, and lungs. Exposure to high concentrations in the air can severely burn the skin, eyes, throat, or lungs. In extreme cases, blindness, lung damage, or death can occur. Breathing lower concentrations causes coughing and nose and throat irritation. Swallowing ammonia may burn the mouth, throat, and stomach.

Anemia: The condition of having less than the normal number of red blood cells or less than the normal quantity of hemoglobin in the blood. The oxygen-carrying capacity of the blood is, therefore, decreased.

Aneurysm: A localized widening (dilatation) of an artery, vein, or the heart. At the area of an aneurysm, there is typically a bulge and the wall is weakened and may rupture. The word "aneurysm" comes from the Greek "aneurysma" meaning "a widening."

Asthma : A common disorder in which chronic inflammation of the bronchial tubes (bronchi) makes them swell, narrowing the airways. Asthma involves only the bronchial tubes and does not affect the air sacs ( alveoli ) or the lung tissue (the parenchyma of the lung) itself.

Blood: The familiar red fluid in the body that contains white and red blood cells, platelets, proteins, and other elements. The blood is transported throughout the body by the circulatory system. Blood functions in two directions: arterial and venous. Arterial blood is the means by which oxygen and nutrients are transported to tissues while venous blood is the means by which carbon dioxide and metabolic by-products are transported to the lungs and kidneys, respectively, for removal from the body.

Blood pressure: The blood pressure is the pressure of the blood within the arteries. It is produced primarily by the contraction of the heart muscle. It's measurement is recorded by two numbers. The first (systolic pressure) is measured after the heart contracts and is highest. The second (diastolic pressure) is measured before the heart contracts and lowest. A blood pressure cuff is used to measure the pressure. Elevation of blood pressure is called "hypertension".

Brain: That part of the central nervous system that is located within the cranium ( skull ). The brain functions as the primary receiver, organizer and distributor of information for the body. It has two (right and left) halves called "hemispheres."

Brain stem: The stemlike part of the brain that is connected to the spinal cord. Or conversely, the extension of the spinal cord up into the brain. The brain stem is small but important. It manages messages going between the brain and the rest of the body, and it also controls basic body functions such as breathing, swallowing, heart rate , and blood pressure. The brain stem also controls consciousness and determines whether one is awake or sleepy.

Brain swelling: See: Cerebral edema.

Breathing: The process of respiration, during which air is inhaled into the lungs through the mouth or nose due to muscle contraction, and then exhaled due to muscle relaxation.

Bronchi: The large air tubes leading from the trachea to the lungs that convey air to and from the lungs. The bronchi have cartilage as part of their supporting wall structure. The trachea divides to form the right and left main bronchi which, in turn, divide to form the lobar , segmental, and finally the subsegmental bronchi.

Cell: The basic structural and functional unit in people and all living things. Each cell is a small container of chemicals and water wrapped in a membrane .

Cerebellum: The portion of the brain in the back of the head between the cerebrum and the brain stem. The cerebellum controls balance for walking and standing and other complex motor functions.

Cerebral: Pertaining to the brain, the cerebrum or the intellect.

Cerebral aneurysm: A localized widening of a vessel within the brain. See: Berry aneurysm.

Cerebral hemispheres: The two halves of the cerebrum, the largest part of the brain.

Cerebrospinal fluid: CSF. A watery fluid, continuously produced and absorbed, which flows in the ventricles (cavities) within the brain and around the surface of the brain and spinal cord.

Chronic: This important term in medicine comes from the Greek chronos, time and means lasting a long time.

Circulation: The movement of fluid in a regular or circuitous course. Although the noun "circulation" does not necessarily refer to the circulation of the blood, for all practical purposes today it does. Heart failure is an example of a problem with the circulation.

Cirrhosis: An abnormal liver condition characterized by irreversible scarring of the liver. Alcohol and viral hepatitis B and C are among the many causes of cirrhosis. Cirrhosis can cause yellowing of the skin (jaundice), itching, and fatigue. Diagnosis of cirrhosis can be suggested by physical examination and blood tests, and can be confirmed by liver biopsy in some patients. Complications of cirrhosis include mental confusion, coma, fluid accumulation (ascites), internal bleeding, and kidney failure. Treatment of cirrhosis is designed to limit any further damage to the liver as well as complications. Liver transplantation is becoming an important option for patients with advanced cirrhosis.

Cocaine: The most potent stimulant of natural origin, a bitter addictive anesthetic (pain blocker) which is extracted from the leaves of the coca scrub (Erythroxylon coca) indigenous to the Andean highlands of South America.

Computerized tomography: Pictures of structures within the body created by a computer that takes the data from multiple X-ray images and turns them in pictures.

Cranial: 1. Pertaining to the cranium or skull. 2. Toward the head. As opposed to caudad. The eye is cranial to the jaw. For a more complete listing of terms used in medicine for spatial orientation, please see the entry to "Anatomic Orientation Terms".

Cranial nerves: Nerves that emerge from or enter the skull (the cranium), as opposed to the spinal nerves which emerge from the vertebral column. Cranial nerves come directly from the brain through the skull.

CT scan: Computerized tomography scan. Pictures of structures within the body created by a computer that takes the data from multiple X-ray images and turns them into pictures on a screen. CT stands for computerized tomography.

Cuts: Severed skin. Washing a cut or scrape with soap and water and keeping it clean and dry is all that is required to care for most wounds. Putting alcohol hydrogen peroxide, and iodine into a wound can delay healing and should be avoided. Seek medical care early if you think that you might need stitches. Any delay can increase the rate of wound infection. Any puncture wound through tennis shoes has a high risk of infection and should be seen by your healthcare professional. Any redness, swelling, increased pain, or pus draining from the wound may indicate an infection that requires professional care.

Diabetes: Refers to diabetes mellitus or, less often, to diabetes insipidus . Diabetes mellitus and diabetes insipidus share the name "diabetes" because they are both conditions characterized by excessive urination (polyuria).

Diagnosis: 1 The nature of a disease ; the identification of an illness. 2 A conclusion or decision reached by diagnosis. The diagnosis is rabies . 3 The identification of any problem. The diagnosis was a plugged IV.

Dura: The outermost, toughest, and most fibrous of the three membranes ( meninges ) covering the brain and the spinal cord . Dura is short for dura mater (from the Latin for hard mother). Also called the pachymeninx (singular) or pachymeniges (plural).

Edema: The swelling of soft tissues as a result of excess water accumulation.

Electrocardiogram : A recording of the electrical activity of the heart. An electrocardiogram is a simple, non-invasive procedure. Electrodes are placed on the skin of the chest and connected in a specific order to a machine that, when turned on, measures electrical activity all overaround the heart. Output is usually in the form of a long scroll of paper displaying a printed graph of activity. Newer models output the data directly to a computer and screen, although a print-out may still be made.

Emphysema: 1) A lung condition featuring an abnormal accumulation of air in the lung's many tiny air sacs, a tissue called alveoli. As air continues to collect in these sacs, they become enlarged, and may break, or be damaged and form scar tissue. Emphysema is strongly associated with smoking cigarettes, a practice that causes lung irritation. It can also be associated with or worsened by repeated infection of the lungs, such as is seen in chronic bronchitis. The best response to the early warning signs of emphysema is prevention: stop smoking and get immediate treatment for incipient lung infections. Curing established emphysema is not yet possible. Because patients don't have an adequate amount of space in the lungs to breathe, they gasp for breath, and may not be able to obtain enough oxygen. Those with severe emphysema usually end up using an oxygen machine to breathe. In some cases, medication may be helpful to ease symptoms or to treat infection in already-damaged lungs.

Environment: The sum of the total of the elements, factors and conditions in the surroundings which may have an impact on the development , action or survival of an organism or group of organisms.

Epidural: Outside the dura, the outermost, toughest, and most fibrous of the three membranes (meninges) covering the brain and the spinal cord. An epidural hematoma is a collection of blood beneath the skull but outside the dura.

Event: A set of outcomes. Cardiovascular events might include a heart attack and gastrointestinal events a GI bleed. The use of the term "event" in medicine comes from probability theory.

Extremity: The extremities in medical language are not freezing cold or scorching heat but rather the uttermost parts of the body. The extremities are simply the hands and feet.

Flexion: The process of bending or the state of being bent. Flexion of the fingers results in a clenched fist.

Frontal: In anatomy, pertaining to the forehead. As, for example, the frontal bone, frontal sinus, and frontal nerve. From the Latin "frons" meaning the forehead or brow.

Glucose: The simple sugar (monosaccharide) that serves as the chief source of energy in the body. Glucose is the principal sugar the body makes. The body makes glucose from proteins, fats and, in largest part, carbohydrates. Glucose is carried to each cell through the bloodstream. Cells, however, cannot use glucose without the help of insulin . Glucose is also known as dextrose.

Heart: The muscle that pumps blood received from veins into arteries throughout the body. It is positioned in the chest behind the sternum (breastbone; in front of the trachea, esophagus, and aorta; and above the diaphragm muscle that separates the chest and abdominal cavities. The normal heart is about the size of a closed fist, and weighs about 10.5 ounces. It is cone-shaped, with the point of the cone pointing down to the left. Two-thirds of the heart lies in the left side of the chest with the balance in the right chest.

Heart attack: The death of heart muscle due to the loss of blood supply. The loss of blood supply is usually caused by a complete blockage of a coronary artery, one of the arteries that supplies blood to the heart muscle. Death of the heart muscle, in turn, causes chest pain and electrical instability of the heart muscle tissue.

Heart muscle: A type of muscle with unique features only found in the heart. The heart muscle, or cardiac muscle, is medically called the myocardium ("myo-" being the prefix denoting muscle).

Heart rate: The number of heart beats per unit time, usually per minute. The heart rate is based on the number of contractions of the ventricles (the lower chambers of the heart). The heart rate may be too fast ( tachycardia ) or too slow ( bradycardia ). The pulse is bulge of an artery from the wave of blood coursing through the blood vessel as a result of the heart beat. The pulse is often taken at the wrist to estimate the heart rate.

Hemoglobin: The oxygen-carrying pigment and predominant protein in the red blood cells . Hemoglobin forms an unstable, reversible bond with oxygen. In its oxygenated state it is called oxyhemoglobin and is bright red. In the reduced state it is called deoxyhemoglobin and is purple-blue.

Hepatic: Having to do with the liver. Pronounced hi-'pa-tik. From the Latin hepaticus derived from the Greek hepar meaning (not too surprisingly) the liver.

Hepatic encephalopathy: Brain dysfunction directly due to liver dysfunction, most often recognized in advanced liver disease. Hepatic encephalopathy may cause disturbances of consciousness and progress to coma .

Heroin: Semisynthetic drug derived from morphine. Discovered in 1874, it was introduced commercially in 1898 by the Bayer company in Germany. The name heroin was coined from the German heroisch meaning heroic, strong. Heroin is stronger (more potent) than morphine.

High blood pressure : Also known as hypertension, high blood pressure is, by definition, a repeatedly elevated blood pressure exceeding 140 over 90 mmHg -- a systolic pressure above 140 with a diastolic pressure above 90.

Hypoglycemia : Low blood sugar (glucose). When symptoms of hypoglycemia occur together with a documented blood glucose under 45 mg/dl, and the symptoms promptly resolve with the administration of glucose, the diagnosis of hypoglycemia can be made with some certainty. Hypoglycemia is only significant when it is associated with symptoms.

Hypoglycemic: Relating to hypoglycemia , an abnormally low level of the sugar glucose in the blood, usually a complication of diabetes, in which the body does not produce enough insulin to fully metabolize glucose.

Hypothyroid: Deficiency of thyroid hormone which is normally made by the thyroid gland which is located in the front of the neck:

Inflammation: A basic way in which the body reacts to infection , irritation or other injury, the key feature being redness, warmth, swelling and pain . Inflammation is now recognized as a type of nonspecific immune response .

Injure: To harm, hurt, or wound. The word injure may be in physical or emotional sense. Treadmill machines may injure anyone who gets on one. Sexual molestation injures children. From the Latin injuria meaning injury.

Injury: Harm or hurt. The term "injury" may be applied in medicine to damage inflicted upon oneself as in a hamstring injury or by an external agent on as in a cold injury. The injury may be accidental or deliberate, as with a needlestick injury. The term "injury" may be synonymous (depending on the context) with a wound or with trauma.

Insulin: A natural hormone made by the pancreas that controls the level of the sugar glucose in the blood. Insulin permits cells to use glucose for energy. Cells cannot utilize glucose without insulin.

Involuntary: Done other than in accordance with the conscious will of the individual. The opposite of voluntary.

Kidney: One of a pair of organs located in the right and left side of the abdomen which clear "poisons" from the blood, regulate acid concentration and maintain water balance in the body by excreting urine. The kidneys are part of the urinary tract. The urine then passes through connecting tubes called "ureters" into the bladder. The bladder stores the urine until it is released during urination.

Lightening: Not to be confused with a discharge of atmospheric electricity, lightening refers to the sensation that a pregnant woman feels when the baby drops. This is the time when the presenting (lowermost) part of the fetus descends into the maternal pelvis.

Liver: An organ in the upper abdomen that aids in digestion and removes waste products and worn-out cells from the blood. The liver is the largest solid organ in the body. The liver weighs about three and a half pounds (1.6 kilograms). It measures about 8 inches (20 cm) horizontally (across) and 6.5 inches (17 cm) vertically (down) and is 4.5 inches (12 cm) thick.

Lungs: The lungs are a pair of breathing organs located with the chest which remove carbon dioxide from and bring oxygen to the blood. There is a right and left lung.

Medication: 1. A drug or medicine. 2. The administration of a drug or medicine. (Note that "medication" does not have the dangerous double meaning of "drug.")

Metabolism: The whole range of biochemical processes that occur within us (or any living organism). Metabolism consists both of anabolism and catabolism (the buildup and breakdown of substances, respectively). The term is commonly used to refer specifically to the breakdown of food and its transformation into energy.

Molecule: The smallest unit of a substance that can exist alone and retain the character of that substance.

Motor: In medicine, having to do with the movement of a part of the body. Something that produces motion or refers to motion. For example, a motor neuron is a nerve cell that conveys an impulse to a muscle causing it to contract. The term "motor" today is also applied to a nerve that signals a gland to secrete. Motor is as opposed to sensory.

Muscle: Muscle is the tissue of the body which primarily functions as a source of power. There are three types of muscle in the body. Muscle which is responsible for moving extremities and external areas of the body is called "skeletal muscle." Heart muscle is called "cardiac muscle." Muscle that is in the walls of arteries and bowel is called "smooth muscle."

Naloxone: A drug that antagonizes morphine and other opiates. Naloxone is a pure opiate antagonist and prevents or reverses the effects of opioids including respiratory depression, sedation and hypotension. Sold under the brand name of Narcan and in combination with buprenorphine as Suboxone.

Narcan: Brand name for naloxone.

Narcotic: 1. A drug that causes insensibility or stupor. A narcotic induces narcosis, from the Greek "narke" for "numbness or torpor."
2. A drug such as marijuana which is subject to regulatory restrictions comparable to those for addictive narcotics.

Nerve: A bundle of fibers that uses chemical and electrical signals to transmit sensory and motor information from one body part to another. See: Nervous system.

Neurology: The medical specialty concerned with the diagnosis and treatment of disorders of the nervous system -- the brain, the spinal cord, and the nerves.

Occipital: 1. Pertaining to the occiput, the back of head.
2. Located near the occipital bone as, for example, the occipital lobe of the brain.

Organ: A relatively independent part of the body that carries out one or more special functions. The organs of the human body include the eye, ear, heart, lungs, and liver.

Oxygen: A colorless, odorless and tasteless gas that makes up about 20% of the air we breathe (and at least half the weight of the entire solid crust of the earth) and which combines with most of the other elements to form oxides. Oxygen is essential to human, animal and plant life.

Pain: An unpleasant sensation that can range from mild, localized discomfort to agony. Pain has both physical and emotional components. The physical part of pain results from nerve stimulation. Pain may be contained to a discrete area, as in an injury, or it can be more diffuse, as in disorders like fibromyalgia . Pain is mediated by specific nerve fibers that carry the pain impulses to the brain where their conscious appreciation may be modified by many factors.

Pancreas: A fish-shaped spongy grayish-pink organ about 6 inches (15 cm) long that stretches across the back of the abdomen, behind the stomach. The head of the pancreas is on the right side of the abdomen and is connected to the duodenum (the first section of the small intestine). The narrow end of the pancreas, called the tail, extends to the left side of the body.

Paralysis: Loss of voluntary movement (motor function). Paralysis that affects only one muscle or limb is partial paralysis, also known as palsy; paralysis of all muscles is total paralysis, as may occur in cases of botulism.

Parietal: Adjective from the Latin "parietalis" meaning "belonging to the wall" that the ancient anatomists used to designate the wall, as of a body cavity.

Physiology: The study of how living organisms function including such processes as nutrition, movement, and reproduction.

Pneumonia: Inflammation of one or both lungs with consolidation. Pneumonia is frequently but not always due to infection. The infection may be bacterial, viral, fungal or parasitic. Symptoms may include fever, chills, cough with sputum production, chest pain, and shortness of breath.

Poison: Any substance that can cause severe distress or death if ingested, breathed in, or absorbed through the skin. Many substances that normally cause no problems, including water and most vitamins, can be poisonous if taken in too large of a quantity. Poison treatment depends on the substance: if there are treatment instructions on the substance's container and you are sure it contained no other item, follow those directions immediately. Always contact your nearest Poison Control Center if you are concerned about possible poison ingestion.

Potassium: The major positive ion (cation) found inside of cells. The chemical notation for potassium is K+.

Prognosis: 1. The expected course of a disease .
2. The patient's chance of recovery.
The prognosis predicts the outcome of a disease and therefore the future for the patient . His prognosis is grim, for example, while hers is good.

Protein: A large molecule composed of one or more chains of amino acids in a specific order determined by the base sequence of nucleotides in the DNA coding for the protein.

Red blood cell: The blood cell that carries oxygen. Red cells contain hemoglobin and it is the hemoglobin which permits them to transport oxygen (and carbon dioxide). Hemoglobin, aside from being a transport molecule, is a pigment. It gives the cell its red color (and name).

Resolution: In genetics , resolution refers to the degree of molecular detail on a physical map of DNA , ranging from low to high.

Scan: As a noun, the data or image obtained from the examination of organs or regions of the body by gathering information with a sensing device.

Sensation: In medicine and physiology , sensation refers to the registration of an incoming ( afferent ) nerve impulse in that part of the brain called the sensorium , which is capable of such perception. Therefore, the awareness of a stimulus as a result of its perception by sensory receptors. (Sensory is here synonymous with sensation.)

Skin: The skin is the body's outer covering. It protects us against heat and light, injury, and infection. It regulates body temperature and stores water, fat, and vitamin D. Weighing about 6 pounds, the skin is the body's largest organ. It is made up of two main layers; the outer epidermis and the inner dermis.

Spinal cord: The major column of nerve tissue that is connected to the brain and lies within the vertebral canal and from which the spinal nerves emerge. Thirty-one pairs of spinal nerves originate in the spinal cord: 8 cervical , 12 thoracic , 5 lumbar, 5 sacral, and 1 coccygeal. The spinal cord and the brain constitute the central nervous system ( CNS ). The spinal cord consists of nerve fibers that transmit impulses to and from the brain. Like the brain, the spinal cord is covered by three connective-tissue envelopes called the meninges . The space between the outer and middle envelopes is filled with cerebrospinal fluid ( CSF ), a clear colorless fluid that cushions the spinal cord against jarring shock. Subarachnoid: Literally, beneath the arachnoid, the middle of three membranes that cover the central nervous system. In practice, subarachnoid usually refers to the space between the arachnoid and the pia mater, the innermost membrane surrounding the central nervous system.

Subarachnoid hemorrhage: Bleeding within the head into the space between two membranes that surround the brain. The bleeding is beneath the arachnoid membrane and just above the pia mater. (The arachnoid is the middle of three membranes around the brain while the pia mater is the innermost one.)

Subdural: Below the dura, the outermost, toughest, and most fibrous of the three membranes (meninges) covering the brain and the spinal cord. An subdural hematoma is a collection of blood beneath the dura.

Temporal: 1. Pertaining to time, limited in time, temporary, or transient.
2. Pertaining to the temple region of the head. The temporal lobe of the brain is located beneath the temple.

Tendon: The tissue by which a muscle attaches to bone. A tendon is somewhat flexible, but fibrous and tough. When a tendon becomes inflamed, the condition is referred to as tendinitis or tendonitis. Inflamed tendons are at risk for rupture.

Thermostat: A device that monitors temperature and automatically maintains it at certain levels. In a human, a tiny part of the brain called the hypothalamus, located behind the eyes, serves as the thermostat. It can warm the body by causing it to shiver and cool the body by causing it to perspire. The hypothalamus also regulates hunger, thirst, sex drive and other body activities.

Tomography: The process for generating a tomogram , a two-dimensional image of a slice or section through a three-dimensional object. Tomography achieves this remarkable result by simply moving an x-ray source in one direction as the x-ray film is moved in the opposite direction during the exposure to sharpen structures in the focal plane, while structures in other planes appear blurred. The tomogram is the picture; the tomograph is the apparatus; and tomography is the process.

Tubes: The "tubes" are medically known as the Fallopian tubes. There are two Fallopian tubes, one on each side, which transport the egg from the ovary to the uterus (the womb). The Fallopian tubes have small hair-like projections called cilia on the cells of the lining.

Unconscious: 1. Interruption of awareness of oneself and one's surroundings, lack of the ability to notice or respond to stimuli in the environment. A person may become unconscious due to oxygen deprivation, shock, central nervous system depressants such as alcohol and drugs, or injury.
